Eficacia del cannabis medicinal en el control del dolor neuropático crónico en adultos: revisión sistemática de la literatura

Resumen

Introducción: El cannabis medicinal ha emergido como una alternativa terapéutica para el manejo del dolor neuropático crónico, especialmente en casos refractarios a tratamientos convencionales. Objetivo: Evaluar la eficacia y seguridad del cannabis medicinal y sus derivados cannabinoides en el manejo del dolor neuropático crónico en adultos, mediante una revisión sistemática de la literatura con el protocolo PRISMA 2020. Metodología: Se realizó una revisión sistemática de estudios publicados hasta diciembre de 2024 en bases de datos electrónicas (PubMed/MEDLINE, PubMed Central, Biblioteca Cochrane, SciELO,). La formulación de la pregunta de investigación se basó en el marco PICOC. Los operadores booleanos guiaron la estrategia de búsqueda, y la calidad metodológica fue evaluada mediante las herramientas Cochrane RoB 2 y ROBINS-I para ensayos clínicos aleatorizados y estudios observacionales, respectivamente. Resultados: De 300 registros identificados, 26 estudios primarios cumplieron los criterios de inclusión: 12 ensayos clínicos aleatorizados, 8 estudios observacionales y 6 descriptivos de contexto regulatorio-clínico. La evidencia indica que el cannabis medicinal produce una reducción moderada pero estadísticamente significativa del dolor neuropático crónico (escala NRS/EVA), con mayor eficacia en pacientes refractarios al tratamiento convencional. Los cannabinoides THC y CBD demostraron eficacia comparable a analgésicos de segunda línea y menor riesgo de dependencia que los opioides (RR = 1,65; IC 95 %: 1,34–2,04). Los efectos adversos más frecuentes fueron leves a moderados y reversibles; los eventos graves fueron infrecuentes (< 3 %). Conclusiones: El cannabis medicinal representa una alternativa terapéutica válida de tercera línea para adultos con dolor neuropático crónico refractario, bajo prescripción médica supervisada. Se requieren ensayos clínicos de mayor duración, con muestras más amplias y mayor estandarización metodológica para consolidar recomendaciones clínicas basadas en evidencia de alta calidad.
